Suspended account due to phishing

Hello my account was blocked due something or I dont know what is wrong some about phishing in my account what is happen ?

Your account is currently suspended. Your form is asking for login credentials like the username and password. This is not allowed in JotForm and it is considered as a phishing form. It violates our terms of use.

We can re-activate your account but we need to delete that form. Thank you.
