Adobe eSign widget not working - error message when sign button is clicked

EchoSign widget was working fine then stopped working. Says it failed when it is clicked on 

One thing that could cause this error is if the Widget was not authenticated. If it is not the case, please try re-authenticate the Widget to see if that fixes the problem.
