Form Disabled

Hi!  I just created my first form and now it says "Form Not Found...This Form Has Been Disabled"

How do we correct this?

I would like to apologize for any inconvenience. I have checked the form and found out that it has fields that collect sensitive information such as credit card details that violates our Terms of use. Unless those fields are deleted, the account will be on auto-suspension status and will not be accessed and your forms will remain disabled.
