I checked your form and the "Get PDF" option downloaded data in PDF format:
It should definitely not be downloading in docx format. Is it still doing so on your end? If so, please let us know what format it is downloading in and we will be happy to have another look.

I tried again and it is still doing it as a word document.

Now when I save it to my desktop the icon is the Word icon, I then right click and I have the option of opening it with different programs and Adobe is one of the choices. I just never had to do this before today, it would automatically save to my computer using Adobe/PDF.

It automatically saves as a PDF. The default application for that file type may have changed to Word instead of Adobe reader. The PDF should open in either one without issue and the format should be PDF regardless.
