when my custom sign request form is submitted i get a pop up stating the requested resource is no longer available...where did my redirect button go and why did this breakPage URL:

I've tried submitting the form and see that the page is redirected to the same form again. I've not received any error message after submitting the form. Unfortunately, it looks like the screenshot attached to the post is not uploaded corrected and not displaying. If you are still experiencing the issue, please try submitting the form in a private/incognito window or a different browser.
