My Forms are disabled!

Please explain why my forms aren't working anymore. I'm losing business. Thanks.

I'm confused what is phishing form and where was this particular form used?

A phishing form is a kind of form which purpose is to mine or collect data for the purpose of fraudulent activities such as a form portraying as a login form which actually is not.

An example of that form is like the form you had which is asking for a username and password, which of course, your form is not an actual login form. It only collects usernames and passwords. Another example is a form trying to portray itself as a payment form for transaction, asking for credit card information including the pin code, etc. when in fact they are not processing payment but only collecting information.

I'm still unaware of where this particular form was used. Thats what is confusing. I don't recall where or when it was used. I'm concerned about this. Is there a way track it and the submissions to see where it was used?

And if it is the case, it was not used in any particular case, as the phishing filter immediately detected it and suspended the account.
