Accidently deleted a form

I Accidently deleted a form, can you help? It was Donation Request form.

Once the form is deleted form My Forms folder it will be moved to Trash folder. It is possible to restore form from Trash folder to My Forms folder. Inside of this guide you can find how to do it:

However if the form is Purged, it is not possible to recover it. 

I have checked Trash folder on your account and I did not found Donation Request form. If you Purged form from Trash folder, then it is not possible to restore it. You will need to re-create it from scratch.
