    I am attempting to use your form but cannot seem to activate it? Can you please advise! -Michael

    I have reactivated your account, but I have deleted the said fields in the form. Please refrain from adding it again. If you need it, you may consider asking it through email. You can setup an auto responder then explain there why you need the info and ask them to send it.
