I have created a form, but when I see the pdf copy it does not show all the information. I have setup google drive for receiving pdf copy, but I can't see all the information in the pdf copy including signature. Could you please look into this , and let me know how to fix this? Thanks

This is because you have edited the PDF report and removed those fields, I downloaded the PDF file once and noticed that; however, I restored the PDF layout and all the fields are included now, I downloaded and I can confirm this.
You are also able to restore the PDF file if necessary, you only need go to the submissions page and click on Get PDF-> Customize PDF.
Once there you only need to click on the Reset button, but you do not have to save the changes, this will restore the layout of the PDF file and it should show all the fields:
