How can I find out the web page the request come from?

How can I find out which page on my website a request came from when I use the same form for multiple pages.

The REQUEST A SAMPLE button opens your form.  I use the exact same form for multiple blogs.

This is possible by using a widget, here are two that will help you to accomplish what you want: 

Get referrer

Get form page url
