Is the submissions quota per form?

No. The submissions limit is for the account itself. For example, if you have 5 forms in your account receiving 20 submissions each, the submission counter will be 100 submissions. You can check the account status by clicking on the User icon on the top-right corner after you logged in to your account or from the Usage section in your account settings.
