My Form Is Disabled

    We are testing a JotForm form to record phone orders for internal operators and everything was working fine yesterday. 

    However, when we open the URL today, we keep getting a page that says the form is disabled. We have tried enabling/disabling the form but still getting the same page. Can you please let us know why this is happening as we intend to use the form today

    Hello muber,

    Your account was suspended due to violation of terms and conditions of JotForm. Specifically, your form appears to have a field collecting for credit card numbers.

    For the meantime, I have reactivated your account. However it will not take long until our system's filter will be suspending you again. Please do remove the said fields pertaining into collecting sensitive information as soon as possible in order for your account to be safe from getting suspended again.
