I have lost my form

    Hi Jotform:

    I created a form yesterday as a "guest" - just to check things out.  The form turned out perfectly - I am delighted with the results. However, I can't find it - HELP !!

    The form is called ADHD Intake Form and I copied and saved the Source Code - <script type="text/javascript" src="//form.jotform.ca/jsform/20615879588268"></script>  and the Embed Code - http://form.jotform.ca/form/20615879588268 because I wanted to show it to a colleague.

    Can you help?

    hi Bonnie,

    although i am just new here at Jotform, I can access you form just fine.. 


    i think your form still there.

    maybe you just need to register officially, its free and would take less than a minute to do.

    Hi Bonnie,

    It would be easier for you to manage your forms if you have a registered account at JotForm. I would suggest to create an account using the same email address that you'd entered on your Form Notification, so the form will be carried over to your new account.
