Form loading strangely on mobile/tablet

    Hello,

     

    We have a form that is loading oddly across multiple devices.  See attached files but basically im trying to view on a tablet and in portrait mode the date and time fields load very strangely as well as the formatting in the insurance area (the fields to fill in info become jumbled).  How can we address this?  PS tried to give an attached example of loading properly in landscape but can only attach one file here, file is at https://drive.google.com/file/d/0B1uKORK8Pqn_MkR3Ump5anUzcVU/view?usp=sharing

    Please keep the responsive enabled in the design.
