When filling in a form on our website it is not being send correctly to our email.
Some fields ar double or don't show any data.
Here you see may holiday and summer holiday, I filled in summer holiday and a date.

I did check the notification email template of your form and found that it is missing few form fields. Please check the screenshot below:
It seems you have edited your email alert. Please be noted that if your email alert is edited, it becomes dirty. That means any changes you make in your form will not reflect in the email alert. You will have to manually edit the email alert template as well.
The best and easiest way to solve this problem is to delete the existing email alert you have in your form and add a new one. Please be noted that a new email alert will always include all fields of form. The following guide should help you:
