Here are the steps you should follow to edit your form and add new fields:
#1. Sign in to your JotForm account.
#2. Click on the "My Forms" link for top menu.
#3. Select the desired form you want to edit and then click on the "Edit Form".
#4. It will load the form in editor and you should be able to edit the form.
#5. You can add or delete the desired form fields from your form. Please check the screenshot below:
