Why doesn't my entire form show on my website?

I have created a form from a blank template. It looks fine ... I hit "publish", copy the link. I then go to my website, add a jot form to a page, paste the link ... then what? I hit enter and nothing happens. If I hit add form, then I have to choose a template. If I try to create a new form directly via the website, it only shows the top portion, not the full form.
