Is it possible for the PDF submission of my form to include the entire text, rather than just the data that was entered?Asked by ccurlin on April 13, 2016 at 03:41 PM
I am using Jotform to load intake paperwork for my mental health practice, e.g., consent for treatment, agreement to our rates, etc. The clients will sign the form, and I will upload it to our electronic record system. I have entered text in the document (our rates, what they are agreeing to, etc), but the only data I receive back is their signature. I would like the entire form to be uploaded, which will show what they actually signed and agreed to.
