Is it possible for the PDF submission of my form to include the entire text, rather than just the data that was entered?

    I am using Jotform to load intake paperwork for my mental health practice, e.g., consent for treatment, agreement to our rates, etc. The clients will sign the form, and I will upload it to our electronic record system.  I have entered text in the document (our rates, what they are agreeing to, etc), but the only data I receive back is their signature. I would like the entire form to be uploaded, which will show what they actually signed and agreed to.

    While viewing your submissions, there is an option to include headers and text in your submissions:
