It seems that your question is posted empty with any information. If you are looking to add radio buttons to your JotForm, you may add the radio button field from the Form Tools section.
If you are looking for some fancy designed radio buttons, you may take a look at our widgets section.

I accidentally sent you a question because I was searching for some information about adding the same design to "other" radio button through CSS.
Just ignore my message, I've solved my problem with a workaround (added new text box and set it to show that text box if radio button named "other" is selected)!
