Hello,
I have to share a survey about the typical day of young people.
I need some information (colomns of the attachment) linked to the hours. In the table, people can choose different answer. The problem is the format but also the collection of the data. It would be clear and ergonomic for our responding but after, we must be able to analyse these data. Would you have a solution?

Hello,
Survey form is possible with JotForm. Since I could not view the attached screenshot, I can only assume that you want a table formatted fields where users can enter their data, select choices within the table. If so I would suggest using Configurable List Widget http://widgets.jotform.com/widget/configurable_list. This allows you to define the various field types per column on the list. Users can also add more fields dynamically as they pleased.
You will also find the Survey Tools helpful.
Your form data can be exported into Excel and CSV for a more ways for analyzing your form data.
