Why is my form disabled?

Hi,

I just updated my Form - Tarantula CRM customizations form - here is the link:  https://form.jotform.com/60804954142151  and it says "form not found - this form is disabled"

Please help!

This happens when our anti-phishing system detects that you are asking for information that is not allowed on JotForm, then your forms are disabled and your account is suspended; however, I have double checked your forms and I can see they are clean and you are not asking for information that is not allowed on JotForm, this is why I have activated your account and re-enabled your forms. 

Please confirms us that they are working fine on your end now. 

Also, please take a look to our terms in order to avoid your forms to be cataloged as phishing and suspend your account due to that.
