I've checked your form and I see that you haven't created an email notification yet. Please create a notification or autoresponder emails so that you can configured the sender email or reply to email. Please check these guides that will help you set up a notification or change the email recipient:
Also, if you use the "Test Email" button in the email wizard, the notification will be sent to the account's email address.
