I am trying to create a matrix with two answers from drop down menus.  I need the drop down options to be different for each column.  Each line of the matrix needs to be a defined question with the two answers. Is there a way to do this?

Yes, you could do this using the 'Matrix' field. You will need to set the Dropdowns value from the Matrix properties in Form Builder.

No I would like the first column to have the following options in the drop down:

Excellent Good Average Mediocre Non existent

And then for column 2 to have the following options:

Very high High Low Very low

Is there a way to do this?

I'm not actually sure this is possible. Since the two columns are related to the rows on the left their values are assumed to be the same.

If the values of the Dropdowns need to be different and only two columns are needed, you could use a Dropdown with their separate options.
