Account suspended

    Help!  Our account seems to be suspended and I don't know why.  Please help us get online.

    That is because your form is asking for credit card details using our plain text fields. Please be informed that we don't allow this type of form in our system. WE consider that as phishing which is against our Terms of Use (http://www.jotform.com/terms/). I have reactivated your account, but I have deleted the said fields. Please refrain from adding those again.

    If you want to collect a payment at a later date, you should use our Payment Integration then enable authorization mode. Here's how: https://www.jotform.com/help/292-How-to-Enable-Payment-Authorization
