One of your forms has been cataloged as phishing form http://www.jotform.us/form/51246799354163; however, I have checked it and noticed that it is clean, I have re-activated your account and you should be able to load your forms again.
Please also read our terms in order to avoid asking for information that is not allowed for JotForm, this will help you to avoid get your account suspended in the future.

Hi my forms got disabled again. It's very frustrating and I can't have this problem from happening again. What would I need to do? Do I need to have paid subscription?
