    Adding additional form fields to form using conditions

    I need to do this on a form. I have copied the clone web address. Where and how do I add it to my template in JotForm so that I can use it?

    We would like to apologize. I understand that you already cloned the form of my colleague, Elton, from the original thread to your account. I can see the form in your account as I have checked. However, it doesn't serve as a template and if you want to apply it to your form, we will be glad to help you with it. Just let us know the requirements you need for the rules of the condition and also the form where you would like it to be applied.

    On the form above, you will notice that if the one filling the form wants to add another student, he will just need to select "Yes" from the radio button field.

    Yes Mike_G, I want a form that will be similar to the link you had me look at.

    I just uploaded my form to this address: http://sutherlandathletics.org/family_registration_form.html

    It is a sports registration form for families that have more than one child that might be interested in joining a sports team at the school.

    I have the parents information, emergency information at the top of the form followed by Student Athlete #1. That information consists of the child's name, date of birth, gender, current grade level, and the sport option available. I also have the medical information area specific to that child. Next for the child is the Release form which is required and also ties into the PayPal feature. I have not added the PayPal yet to this page.

    After the parent registers the 1st child there is a check box to select a 2nd child. This is where I need help. I tried doing a drop down box, but could not figure that out. Then I saw the "adding additional fields" and thought that this might help. 

    What I ultimately want is a form that the parents can fill out their details first and then add 1 to 4 total children, with one final payment to PayPal at the end.

    Is this doable?

    I must be getting tired - hit submit without signing. I will look for your response in the morning.

    I have cloned your form and first noticed that you have some conditions that are not properly set. 

    Seems like you tried to make this using other fields and you deleted them; however, I will work using the fields that you currently have on your form, the check boxes. 

    You only need to make the condition and ask, for example, do you want to add a  second child? if this is answered with yes then you will show the other fields, in this case as you need to show more  than one field you should select the action "Show Multiple", here is an image that will help you to better understand what I'm talking about: 

    As you are setting up the condition, the fields "My child..." and "Do you want to add a 3rd child?" will be hidden immediately and they will be shown only if the question "Do you want to add a 2nd child?" is answered yes.

    I have made the corrections and finished filling out the template to cover all of the requirements for this Multiple Children in a Family Registration. 

    I need help with the PayPal payment. I have it set up with the two choices for child 1, child 2, child 3, & child 4. 

    On the forms that register just one child I included a Hidden Box then in the properties box of PayPal I added the "My child - Student Athlete" to the Custom Field. We needed the student name so that the Treasurer would know which student & sport the payment was for. Does that make sense. Anyway, it works nicely when registering one student. How do I do this when there could possibly be as many as 4 children registered on the Family Registration Form. The students would not be playing in the same sport because they would be different ages, I would think. However, there could be some twins at the school, but no triplets or quads.

    I did add a hidden dos but it is only referring to student athlete #1. I really need to have the option of all 4 names being tied to the sport chosen.

    Help me figure this out so that the treasurer's job does not get confusing as it did last year, which was our first year doing this.

    My suggestion would be the following:

    1) Add a Form Calculation widget. 

    2) Include the fields where the names are inputted.



    3) Hide that field and assign it in the Paypal option:

    Is the calculated field hidden?

    Do you leave the name "calculated" or do you change it to Student Names?

    I haven't hidden it, but after you make sure the values are passed, you can hide it:

    I changed the label from "calculation" to "Student Names", simply click on the label and change it to whatever you want to.
