    Change the title or subject line of the form's email notification based on a template?

    Lastly - I have used one of your templates (massage therapy) and everytime I get a submission on that form it says "massage therapy intake form". Is there anyway to change that? Or do I just need to create a blank form from scratch.

    I presume you are referring on the email you received from the form titled "DCC Coaching Agreement" form?

    You can edit the email notification under "Emails" and click the "Edit" button.


    You should be able to see the "Massage Therapy Client Intake" title in the email template editor. There you can edit, customize, delete or modify it.


    Or you can simply create a new email notification, that should automatically add the form's title in your email template.
