None of my forms are working - disabled

    None of my forms seem to be working.

    They all appear to have been disabled.

    I can see the forms when I log into my account however when I try to send a link to someone or someone tries to contact me via one of my forms, they receive a message that the form is no longer available / disabled.

    Please advise.

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I've checked your JotForm and see that you are collecting Credit card information using plain text fields on your form.

    It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on Phishing forms.

    We can activate your account only if you agree with our terms and remove the guilty fields from the form. Please let us know so that we can delete the fields and activate your account.

    Those fields have been removed.

    I do however collect the last 4 digits however not the entire number as needed by my company for liability.

    Is collecting the last 4 digits also in violation?

    I'm going through the terms of usage but did not see that specifically.

    I definitely do not want to break any rules.

    As mentioned, we do not allow collection of sensitive information including credit card information, even though it is just the last 4 digit. Our anti-phishing detector will still see this as suspicious and will most likely suspend your account again. I see your form is still collection credit card information:

    In our terms page, under the "Phishing" section, this is what is stated:

    Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

     

    If you need to ask payment from your users, it is a must to use our payment tools or integrations. If you do not wish to charge them immediately, then you can enable "Authorization Only" in the payment options.
