Account suspended, please rectify

Hi,

My account has been suspended. I am not aware of any wrongdoings or operations that would warrant such a measure.

Can you please let me know why this has happened and how it can be reversed?

You were banned because you have a form that is asking for sensitive information like username and passwords. Here's a screenshot of the form in question:

These forms clearly violates our terms of use. We can re-activate your account but we will need to delete the form that violated our terms and you must agree to avoid creating similar forms.
