Its missing several questions all together
missing titles on email submission making it hard to see whats what...
its not including any drop menu options
Several problem to fix. Im a designer and this form is on one of my clients websites... need to fix asap...

It seems that you have already modified your email alert. Please take note that the emails of an email will no longer update once you have edited it. So any additions of fields after this will no longer update your email alert.
What you can do is add a new one and replace the old. To do this, simply edit your form and click Email > Add New Email
After this select Notifier. Proceed and finish the wizard.
This should create a new email notifier for you and you may delete the old one.
If this doesn't fix the problem please do let us know.
