The challenge I'm having is that the configurable list categories under "Child's Info" extend too far to the right and thus are cut off. The original JotForm thread where I had posted my question had created custom CSS to essentially wrap the text onto 2 rows/lines so that all of the categories are visible.
I copied the CSS and thought about muddling my way through trying to adjust it myself but I don't know the first thing about CSS so I thought I would at least ask and see if someone on your end might be able to do it much faster (and correctly!) for me???

Thank you for providing us with the link to your form and the additional details about the css problem you are experiencing.
I can certainly see the issue on your form with it being cut off.
