I have checked you account and it is active. Also all of your form are working.

If your forms are still not showing on your websites, please provide us URL of your websites to that we can check them there.

You do not need to provide us URL's of all of your websites. You can provide us a few URL's.

    Looking forward to your reply.

https://dentalcareofchinohills.com/appointment/ we have an appointment form in this page. Also there is a contact form in the sidebar of each page of this site.

Same issue with this page https://aladindds.com/request-an-appointment/ .

There are more. http://s31.postimg.org/4zx9t0z6j/screenshot_1627.png this is what I see when I try to view any form.

    Hello DevOps,

I did check your webpage which you have shared and found that the form which is embedded in webpage is not there in your account. It is actually associated with account "uberspaceguru". This account is currently over quota and all form are disabled. That is the reason why your forms are not appearing in your webpage.

It seems based on your email request, your silver yearly subscription was transferred to account "uberspaceguru"  from "DevOps". I have taken care of your account issues and upgraded your account to silver yearly plan. Your account is now active and all your form are back online.

    Thank you!
