We set up a questionnaire called 'The Branding Retreat' last week. We received one submission and realized that we needed to gather their name and email address. So a couple of days ago I added the name and email addresses field to our questionnaire.
But today we received a submission and there was no name and email address filled out.
Could someone please look at our form and see what could be the issue.
We really need the name and email address of our participants.

I saw that 5 hours ago you made your email and name fields to be required. Before that they were not required and probably your user did not enter any data inside of those fields.
I have also tested your form and I was not able to submit it without filling email and name field. After that I have submitted your form and I checked our email log. My submission was sent successfully to your email address with all data that I have entered.
Considering that your email and name fields are now required, you users will not be able to submit your form without filling email and name field.
