I have been looking for a form in here for my counseling notes..Any recommendations?

That is an interesting question. Are you looking for a form that you can enter counseling notes into after each session? I was thinking about that for my private coaching practice but wasn't sure how notes would be separated according to each client. I"ll be curious to see if anyone has any suggestions.

The easiest way to separate by client would be to have a fields to enter client information. A name field or even a number field if you have a specific ID number for each client. While viewing your submissions, you would be able to sort your submission data based on that fields entries.
