Fields suddenly not showing on a form

Hi Jotform,

my form dissapeared from my account and wont show anymore in the browser.

I already cleared the caches of my browsers and in the Jotform account.

Still no effect..

Could u please help me?

I would suggest you go the Revision History of the form so you can revert it back to its original stage before its form fields were removed.
