Matrix Widget: Ability to Check More Than One Column Per Row

I checked the said form and you have a couple of matrix widgets there. When you say, you are "not able to press more than one button", do you mean you cannot check multiple columns in one row? If so, the reason is that the input type of your matrix widget is a radiobutton. To allow checking multiple columns in one row, you need to change its input type to checkbox. Please see screenshot below.

You will then be able to select multiple boxes in one row.
