I had an active form which has not passed 100 submissions, yet has been de-activated. I also cannot log into the account I had. The problem now is that I cannot find the template for the form so I can recreate it - even as a paying customer. Can you please let me know if I can find the template for the form and re-create it?

I checked your account - hireit - and you only have one form.
The form you have given - https://form.jotformeu.com/60473308933356 - belongs to a guest account. You might have created it without logging in. I have now moved the form to your account - hireit - and it should now be accessible.
You said you also cannot login to the account you had. Please try resetting your password.
