All of my work over a 2-hour period was lost. So I reverted the form to a previous version, and luckily I was able to recapture many of the changes.
However, when I now go back to edit the form, the formatting is screwed up. If I go into form designer mode, none of my earlier formatting is preserved. If I make no changes, when I view the live form, it maintains everything I did before. If I make a change with the screwed-up formatting, all of my original efforts are lost.

That is because you've applied a theme from the theme store to your form. The theme design won't load up in the designer so that's why you can see that there. IF you would like to customize the form, I would suggest removing the theme.
In the form builder, simply click the Themes:
You should see the button to remove the theme once you get there.
Let us know if the issue persists after removing the theme.
I never added a theme? And when I go to the Themes page, it doesn't show
me as having a theme applied to my form. See the screenshot below. You
can duplicate the form and see the same thing yourself.
You are correct Kait, there is only a default "Default Theme" theme applied to your form, which should not be causing any issues to your designer.
Now seeing how you have your custom styles applied to it, I presume that the issue might be that they did not get loaded for you, but instead only this default theme is.
Now checking your form and the designer I see that there are two different forms shown on the same.
As such I will raise this to our developers and we will update you in regards to the same as soon as the issue is resolved.

A bug ticket report had already been submitted and now assigned to one of our developer.
We will notify you here once we have news/updates available on the status.
You've now had a week to address the bug with the form. The delay in
getting this figured out is really frustrating.
Can you please tell me when it will be addressed? If not by tomorrow at
noon, then you can stop your work on it, because I need to have the form
ready to go by the end of the day tomorrow. I'll just reserve a few
hours to redo the formatting on the entire form.
Our apologies, but we cannot give you an exact timeframe. One thing is for sure, our developers will notify us through this thread for updates.
A workaround for this would be to create a new form. You can style this new form to your preferred layout. Or, create a blank form and copy the fields from the original form to this blank form using our field manager app. http://apps.jotform.com/app/field_manager.
