Include all form information on Submission

I would like to receive the entire form when someone hits "Submit"

Including areas where there is nothing but the text.

Right now (on the form attached) I only receive the Date, Name, and Signature and I would like to receive the whole form.

Since Notification do not add texts and headers to the email by default you need to add them in the email. You can do that simply by copy and paste the texts from your form in the email notification.
