the size of the attached image is not correct - please make the size proportial to the eye. I made the image in paint and I am not very good at this. Also the submit button is the same as on this page - if you can copy and paste it - i dont know how.

I see the labels are already in color blue. But I am not sure what layout you would like to have? You mentioned that the attached image is not correct. If you want to position your fields in a new line, this guide will help you: https://www.jotform.com/help/328-How-to-Position-Fields-in-JotForm
I am also note sure if you are copy and pasting the codes, I see you have a lot of errors in the CSS code. Please do fix them.
After that, try adding this CSS code at the bottom, this should change the design or style of your submit button:
color: #ffffff !important;
Or you can simply use the Form Designer Tool to redesign the button.

I need some help on this and I waited so long for you to write back with assistance or css code and you didnt even read what I wrote - I MADE an image for you to look at and give me css code for or instruct me on how to do it. and u didnt really do either - please help or tell me how much I hafve to pay for someone to help me out.
U have a great product but this tech support on this thread was not very attentive. I am on a deadline and I just waste like 9 hours waiting for a reply from u.
please help if u can or tell me where else to go.

The guide provided by my colleague should suppose to help you if you have followed it correctly. It has instructions on how to move fields to the next line or beside the other fields.
Anyway, I have made CSS corrections in your form because there are lots of errors on the CSS codes injected into it. I have also styled it according to your screenshot, although it is not part of our task to customize user forms. We do it in our own way.
Please check your form now and let us know if there's anything else you need help with.
