Hello i have got email regarding my account disabled and i replied it also as told me to do so now how long should i wait for reactivate my account i just use on my server for my own purpose for my clients i dont get what is reason and why your system catch me as phishing , waiting your answer , if anything need to do let me know thanksPage URL:

I see that your JotForm titled "Unlockcodesonline Verify Page" is having a field username which is not an allowed term with JotForm. The account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities.
It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on Phishing forms.
I've now activated your account so that you can login to your account and remove/change the guilty field immediately to avoid any further suspensions.
