I checked your form https://www.jotform.com/61014546260951 and I found that the upload field you were using was the Transloadit widget.
You must provide first your Transloadit API key for this widget to work.
If you do not have the the API key, you may want to just try also using the regular Upload field. It should also work on mobile device browser.
Let us know if issue remains.

I see that you've deleted your transloadit widget. We would appreciate it if you could add it on your form. At that time, we can test it and we can analyze the problem. If you have any question or issues, please do not hesitate contacting us. We will be glad to assist you.

Hi,
I tested your form and it looks like the Widget is not authenticated. You need the make sure the Widget is Authenticated by copying the API key from your Tranloadit account.
I also made a clone of your form and was able to upload files both from my computer and Android phone.
If you are testing this on Moto G, you OS is probably Marshmallow 6.0, you will need to grant any app permission to be able to access some features on the phone.

It might happen that your application do not have permission to the phone storage or camera.
I'm not sure if it is really possible to convert your form to an Android application and get all the functionalities working properly without some coding.
If you are using an application to do so, you might need to refer to their documentation for some details.
You could try https://www.appsgeyser.com/ or http://www.appypie.com/ as alternative.
