I'm not able to view your site using any of the endings (.com/.net/etc) and i can't assess my forms on my site.
i'm using firefox & safari on a mac. i'm including a screen shot of the page where the form is supposed to be. it's black. furthermore, i can't even get onto the site.

Try to use the iFrame code instead of the Embedd code , I could easily open it with Chrome.
iFrame code works great in these cases , the only downside is that you need to copy and paste into your site code the code whenever you make changes to your form

No issues viewing the forms here either.
Are you on a corporate or some other kind of local network network?

I'm using the service at my home....on the same network i've used for a year....nothing has changed.....i actually spent the last 45 mintues on the phone with uverse. they deny blocking the site....BUT 5 devices logged onto my uverse account could not access the site at all.....when i logged onto my neighbors wifi (cox), i could view the site & forms on my site. WHEN i could get the site to pull up, i actually took a screen shot.
i'm surprised my post even went through. I emailed support today and didn't hear back.

Your site is fine here. I can view your site and form without any problem. I'm sure it's something to do with your browser or connection settings. Did you tried clearing your browsers cache? Please try to check if your not using proxies. Maybe there are some browser extensions or anti-virus that's blocking JotForm site. Please check everything on your side and let us know if the problem still persist so we'll investigate this further.

uverse says they disabled the firewall on their end. after they did that I could finally see the site. I have no firewall on my mac nor anti virus. Uverse also says that it wasn't on their end but jotform....which i don't believe since I could access the site from various wifi networks near my home that aren't secure.
By the time I was able to post on the forums, the firewall was disabled and I could access the site (the first post was earlier and i'm not sure how it even got thru bc after i typed it the page went blank. Actually wondered if it was because jotform may be more data intensive.....att has been known to throttle accounts.....
so, idk what the real cause was/is....i just hope it doesn't happen again.

Glad to know that it's working now. I've check your website form using Firefox, chrome and IE and all works pretty fine same as the previous comments. Let us know if this happen again.

Just in case anyone has this problem....
I am running AirPort utility & a Mabook pro with Lion. I think I figured part of what was going on....Airport Firmware updated automatically but my AirPort Utility was still outdated and would not update...not even with the automatic update. Once I was able to get the AirPort Utility to update so that it was compatible with firmware, I was able to "see" the jotform sites & form again. This is a weird glitch.
also, i had to hard reset my uverse gateway....that may have played a part also....
