Unwanted captcha upon a form submission

I've tried creating a 'new' form by generating a new Jotform url, so that new submissions would be sent from your server to a dedicated email address (since we have multiple forms). Unfortunately, it results in the unwanted 'Captcha' page, and once the catcha is filled out correctly, it results in a 'blank' page.

And, no. Using the 'iFrame' method won't work, since the form content is heavily edited for appearance, after the initial 'source' code is generated from your end.

This usually happens when you have multiple forms embedded on the same web page, or when the form source code is missing some important parts. I would like to suggest posting a link to the web page with embedded form, so we will be able to check this out.
