    You can add the CSS codes below to your form:

    label[for="input_13_1006"] + br + br + span.form-sub-label-container {

        width: 19%;

        float: right;

        margin: 0;

    }

    #input_13_1006_price {

        display: none;

    }

    label[for=input_13_1006] b {

        display: none;

    }

    label[for="input_13_1006"] + br {

        display: none;

    }

    Once you have added those codes to your form, your form should look the same as this form:

    Since the original form, 61181293397158, is already deleted.

    I have found on your other threads that this is now the form that you're working on,

    http://www.jotform.us/form/61188602679162

    Here are the CSS codes that you need to add to that form to achieve your requirements.

    #cid_13 > span:nth-child(8) > span:before {

        position: relative;

        left: 5px;

    }

    #cid_13 > span:nth-child(10) > span:before {

        content: "$" !important;

        position: relative;

        left: 5px;

    }

    label[for="input_13_1006"] + br + br + span.form-sub-label-container {

        margin: 0 10px 0 0 !important;

    }

    Here's a clone version of that form where I have added the codes above for testing:
