I have deleted all recipients as per your support doc. I then put them back, but still get undefined.
Thais started happening about 2 days ago when I did add a name to the recipient list using the old envelope interface.

I can see what you mean, simply create a new email notification: http://www.jotform.com/help/25-Setting-Up-Email-Notifications
