How do I add a full name, first middle and last?

    To do that, you can add the Full Name field to your form. Once you do, you will be able to add prefix, suffix and middle name.

    I do however want to ask you to only open 1 thread per issue / question. That allows us to properly assist you with the same. It also makes sure that our forum anti-spamming algorithm does not see you as spammer - since you have been opening several threads on the same subject.
