How do I create labels using my registration list?

    My registration form is for an event, and with the form I have chaperone(s) who registered up to 15 participants. How do I create labels with responses?

    If I understand correctly, you want to add labels in the response of your forms? I cloned your form and made a test submission to see the response. All the fields (date, chaperone name, school, etc.) seem to have the labels setup.

    I downloaded the excel sheet with hopes to create conference event badges
    using registered names in lists within the sheet. However in the
    registration form withing list of 15 names. The names are garbled with
    "first" "last," names are separated into groups of names per registration.
    How can make conference badges easier?

    I downloaded the test submission in an excel and I see that the configurable list result is displayed in one column.
