Show/Hide Conditions: Conditions are not working even when the related field and option are selected

MY conditions just stopped working. Tried to duplicate eh form, also tried to redo the entire form. Conditions ARE Correct! never changed them. It just stopped working for no reason

I cloned and checked your form and noticed that there are some conditions that are not responding; however, I see that there may be some conflicts between the conditions in your form since you're hiding some fields and trying to show then at the same time, this will not respond. 

I would suggest you to group the fields based on selections, for example, group 3 or 4 fields using the Form Collapse tool and then use conditions to show and hide them, but be careful to do not affect the same field with more than one condition at the same time, for example, try to only set the condition to show or hide one field and set the field as hidden by default, doing this way you will avoid to hide the field and keep it hidden when trying to show it with other condition, this will simply met only one condition.
