It seems that the issue is now resolved. I am using Google Chrome and I cloned your form. I did a test submission on the cloned form and the image is showing up in the PDF file.
Also, I logged in to your account and download the submission as PDF. The uploaded image is showing properly. Here's a screenshot:
Are you still having issues with it? Can you please try logging out and then logging back in. If the issue persist, can you please try if it will work on another browser or not? Lastly, I will clear your form cache so that your forms will be refreshed from our servers.
